While I'm sure some clients would have perfectly healthy reasons for wanting these letters, the whole concept smacks of the notion of propping up someone's delusion. One of my very good friends is a shrink, and I can imagine she'd have all sorts of things to say about the ethics of such an enterprise. Having said that, this would be <i>perfect</i> NaNo material, and I think you should attack it with gusto.
